Clinical Student Absence Form

This form is to allow students to notify the Office of Academic and Student Affairs when they cannot attend class for any reason. This may be due to illness or hardship, or to request an excused absence for a conference. If you have seen a health care provider, you must provide documentation to Jennifer Hamilton.
